Write Allergy Names

Write medicine names, food names, symptoms, and when the allergy happened. Use both English and Japanese if possible.

Carry the Information

Keep allergy information in your wallet, phone, medicine notebook, or emergency bag.

Tell Every Hospital and Pharmacy

Do not assume a new hospital or pharmacy knows your allergy history. Tell them every time.

For Children

Tell school, nursery, babysitters, and family members. Keep written instructions for emergencies.
